SERVICES:

• Locate and monitor services to meet clients' in-home health needs.
• Assist with referrals to federal and state entitlement programs.
• Keep financial records and assist in paying bills and processing insurance claims; provide overall financial management if requested.
• Locate and monitor alternative living needs, when appropriate.

FEES:

Individual/Family Consultation:

$80.00 per hour, plus travel time when meeting is held outside the Age Connections' office.

Case Management/Assessment:

$80.00 per hour, inclusive of travel time unless otherwise noted. Initial Case Management/Assessment includes one written plan of care report.

Probate Court Appointed Visitor:

$125.00 per hour, plus travel time. Includes written report of findings and recommendation to the Probate Court and all interested parties.

Court Testimony:

$150.00 per hour, plus travel time, preparation time and out of pocket expenses.

Private Care Providers Services

A cost effective in-resident arrangement that provides a licensed Care Provider for 1-hour or more per visit, 1-7 days a week, directed by the client and/or family.
