Age Connections, Inc is a locally owned company devoted to improving the lives of our client's and their family members. For more than 15 years we have been providing private Care Management services and non-medical in-residence Care Providers to individuals who need assistance to allow them to "age in place" with dignity and safety of their environment.   We also offer supportive services to a client with a terminal condition who needs additional in-residence support while receiving services from a Hospice agency.

Our non-medical in-residence Care Providers are available to anyone who would benefit from a personal companion, or homemaker.   We assist the odler adult and physically and/or mentally disabled adult who need additionally care to remain independent in their living environment.

At Age Connections, Inc we offer short or long-term care and companionship provided in the comfort of a current living setting including private homes/condominiums, apartments, assisted living, hospital settings and skilled nursing centers and long-term care residences.  

Our Care Providers can provide service from 1 - 24 hours of care per day, 7 days a week to provide respite care for current family care providers, to supplement care with family members or by providing full or part time care.  

Our professional and licensed staff can provide companionship and basic assistance in activities of daily living, personal hygiene care, toileting, light housekeeping, meal preparation, laundry, transportation needs, medication reminders, and other duties as assigned by the agency and approved by the client.

Age Connections, Inc.'s private non-medical home Care Providers are licensed with the Utah Department of Health and they undergo a vigorous screening, background and reference check before they work for any client.  

They have the compassion necessary to treat each client with respect and dignity!

  • We completely explain to the Care Providers the client's needs, requirements, and any special circumstances

  • We bring the Care Providers to your home the first day of work and introduce the Care Providers to the family.

Sign your Service Agreement to Guarantee your Services

Our service agreement is signed only after the client and other parties involved thoroughly understand the rights and responsibilities under the agreement, including schedules, services and pricing.

  • Your Agreement will help protect you and guarantee your service rate
  • You can get service within 24 to 48 hours or
  • Give us a start date convenient for you and/or your loved one.
Ongoing Visits

Visits to the client's home while Care Provider is present to evaluate:

  • Changing needs of the client
  • Client's satisfaction
Our Promise to You

  • Monthly Check-in Phone calls
  • We understand that the level or type of Care may change from time to time
  • Should you have a need to increase or reduce the type of Care you originally requested or to adjust hours to accommodate your schedule, please inform us and we will make the necessary changes immediately.
